1.	Complete & Submit Team Registration
2.	Send Check for Team Registration, payable to: PLANET FUTSAL, PO Box 3144, Los Altos, CA 94024
	Cost: $550 (before 11/1), $575 (before 11/15), $600 (before 12/1)
3.	Provide a USFF player card and a Planet Futsal player participation agreement, completed & signed, for every player on the roster. The USFF player passes must be presented on the first game day and every game after. The Planet Futsal player participation agreement can be downloaded from the website. The player card and waiver must be provided before players are allowed to participate.
